What is ERP?
If you’ve looked into treatment for OCD, you may have already come across Exposure and Response Prevention (ERP). You might even feel like you’ve tried it before — and that it didn’t work. This is something I hear often.
Can you give me some idea of what ERP is?
Many people come to me having been told to “do exposures,” but were left unsure how to actually do them, whether they were doing them correctly, or why their anxiety never seemed to decrease. Some describe pushing themselves through intense fear, gritting their teeth and trying to “get through it,” only to feel just as anxious afterwards.
Others have been given ERP as homework, without enough guidance or support — left to navigate a difficult process on their own.
When ERP is done this way, it’s not surprising that it feels ineffective.
What ERP Actually Is
ERP is the most effective, evidence-based treatment for OCD. At its core, it involves gradually facing the situations, thoughts, or feelings that trigger anxiety, while learning to respond differently—without falling into compulsions or avoidance.
But effective ERP is not about forcing yourself through fear, or trying to make anxiety disappear.
It’s about learning that you can handle discomfort, uncertainty, and intrusive thoughts—and still move toward the life you want.
Different, Expectations, Goal You might still have OCD even if…
- Most of your compulsions happen in your mind (rumination, analysing, replaying)
- You feel stuck needing certainty or reassurance
- You don’t have obvious outward rituals
ERP can be highly effective for these more internal experiences as well.
A Different Experience of ERP
The way I practise ERP is collaborative, structured, and carefully tailored to you.
You won’t be sent away with a list of exposures and left to figure it out on your own. We work through it together, step by step, so you feel clear, supported, and confident in what you’re doing.
ERP also isn’t something that is done to you.
You won’t be pushed into exposures you’re not ready for, and we won’t take a “sink or swim” approach. Every step is discussed and agreed on, with a focus on building your confidence over time.
You may have heard extreme or sensationalised examples of ERP—things like being made to eat off a toilet seat. That’s not how effective ERP works.
Instead, we focus on meaningful, relevant exposures—things that matter in your life, and that help you move toward the person you want to be.
ERP isn’t just about reducing anxiety—it’s about helping you get your life back, by doing what matters, even when anxiety is present.
What you can Expect
ERP is about gently and consistently building your ability to handle discomfort in the service of something bigger—your freedom from OCD.
That might mean:
• Doing things you’ve been avoiding
• Letting go of rituals or mental checking
• Learning to sit with uncertainty without needing immediate answers
You don’t have to do this perfectly. And you don’t have to do it alone.
Goal
The goal of ERP isn’t to eliminate anxiety completely.
Anxiety is a natural human emotion—it’s part of being human, and it’s okay to feel anxious at times. Rather than trying to get rid of it altogether, therapy focuses on helping you change your relationship with it.
Over time, you learn how to experience anxiety without being dictated by it—so it no longer controls your choices, your time, or your life.
Anxiety doesn’t disappear—but it stops running the show.
This is what allows people to step out of OCD’s cycle and back into living in a way that feels meaningful and free.
